What Matters Most
What People Want at the End of Life
Research consistently identifies five wishes. SGE Care's end-of-life care is built to honour all of them.
What We Provide
Our End-of-Life Care at Home
Personal Care & Comfort
Gentle, careful personal care — washing, mouth care, skin and pressure area management — delivered with complete sensitivity to the individual's comfort and personal wishes.
Symptom & Pain Support
Close monitoring of pain, breathing, agitation and other symptoms — with prompt communication to the palliative care nurse, district nurse or GP when changes occur.
Companionship & Presence
Being truly present. Listening to stories, sitting quietly, reading aloud, playing familiar music, holding a hand. Sometimes the most important care is simply being there.
Overnight & 24-Hour Care
Sleeping night and waking night care ensuring the person is never alone. Live-in care available for continuous round-the-clock presence.
Family Carer Relief & Support
Practical and emotional support for family carers — providing breaks, answering questions, guiding families and offering reassurance.
Practical & Administrative Support
Liaising with healthcare professionals, helping families understand what to expect, supporting advance care plans and ensuring documentation is in order.
How We Support You
How End-of-Life Care Begins
We understand this is one of the hardest things a family does. We make the process as gentle and straightforward as possible.
A Gentle First Conversation
Call us, email us or ask your GP or district nurse to make a referral. There is no pressure — just a compassionate member of our team ready to listen.
An Honest Assessment
One of our most experienced coordinators will visit at home to understand support needs, the individual's wishes and the appropriate level of care.
A Personalised Care Plan
We build a care plan reflecting wishes, clinical needs and the role of family. This is shared with all healthcare professionals and updated as circumstances change.
Care Begins
Carefully selected care workers — matched for gentleness and experience — begin providing care. We ensure continuity and familiar faces wherever possible.
Ongoing Support — Including Bereavement
We maintain contact after bereavement, offer signposting to support services and ensure families feel supported, not abandoned.
The measure of a life is not its length but its quality. Our role is to ensure that the final chapter is filled with comfort, dignity and the presence of those who matter most.

We prioritise continuity in all end-of-life placements — familiar faces provide enormous comfort. We will always introduce any new care worker personally and in advance, and never make changes without discussing them with the family first.

We increase care as needs change — from daily visits to multiple visits, overnight or live-in care as required. We are responsive to changes at any time of day or night. No family should feel they are facing this alone.
Yes. Palliative care can begin from the point of a terminal diagnosis. Many people benefit from our support for weeks or months — receiving practical help, companionship and assistance that improves quality of life throughout the time remaining.
