Dual Diagnosis
Understanding Dual Diagnosis
Dual diagnosis describes the co-occurrence of two conditions where each influences the other, making treatment and support more complex.
Types of Complex Needs We Support
What We Mean by Complex Needs
Learning Disability & Mental Health
The most common dual diagnosis in adult social care. Individuals may experience depression, anxiety, psychosis and personality disorders alongside their learning disability — each must be addressed in a truly integrated way.
Autism & Challenging Behaviour
Challenging behaviour in autistic individuals is almost always communicative. We use PBS to understand the function of behaviour and address underlying causes, rather than managing the surface presentation.
Learning Disability & Epilepsy
Epilepsy is significantly more prevalent among people with learning disabilities and often requires complex, tailored medical support alongside the primary care package.
Mental Health & Substance Use
Dual diagnosis of mental health and substance use requires a non-judgemental, harm-reduction approach — addressing both conditions together in a trauma-informed, recovery-focused framework.
Acquired Brain Injury + Multiple Needs
ABI often presents with a combination of cognitive, physical, emotional and behavioural needs that require genuinely complex, specialist care planning and coordination.
Complex Medical & Behavioural Needs
Individuals with both significant medical care needs and challenging behaviour — requiring care workers with advanced clinical training and behavioural expertise simultaneously.

Frequently Asked Questions
Questions About Complex Needs Support
A placement is complex when the combination, severity or interaction of conditions requires highly specialist skills, robust risk management, intensive MDT coordination and a care team with specific expertise. This includes dual diagnosis, significant challenging behaviour, complex medical needs alongside behavioural needs, or a history of multiple placement breakdowns.
Yes. We actively accept these referrals. We take time to understand what has not worked and why, and design our approach to address those factors. We are transparent about what we can offer and what the realistic challenges are.
Risk management is dynamic, not static. It begins with thorough assessment at placement and is reviewed regularly — particularly after significant incidents. We have clear protocols for specific risk types and all staff are trained in their individual service user's risk management plan.
Yes. Where individuals have been supported using physical interventions, we actively work to reduce these using PBS-led approaches, environmental modifications and relationship-based strategies. Any physical intervention is a last resort, proportionate, documented and reviewed.
For the most complex placements, we typically require a minimum of two weeks to complete assessment, recruit and train the team, adapt the environment and finalise plans. For urgent situations including hospital step-downs, we work as quickly as safely possible while maintaining quality standards.
Complex Needs Require Specialist Expertise. We Have It.
At SGE Care, we do not shy away from complexity. If you are looking for a specialist provider in Essex with genuine experience of dual diagnosis and complex needs, talk to our team today.
